Hip Fracture ICP Programme

Early screening of patients at risk of falls and osteoporosis can help delay or prevent hip fractures.

Our Hip Fracture ICP Programme provides GPs with access to bone mineral densitometry (BMD) scans. Patients at risk of falls or osteoporosis can be screened early for early intervention, if necessary.

If a patient sustains a hip fracture, s/he will be cared for by a multi-disciplinary team of doctors, nurses and Allied Health Professionals to ensure the best possible evidence-based care.

After surgery, patients with good rehabilitation potential will undergo intensive inpatient rehabilitation. They will be discharged afterwards to our community rehabilitation partners to continue the rehabilitation. This will ensure a smooth transition back into the community and minimise dependence on caregivers.

Only direct referrals from JurongHealth Campus' GP partners will be accepted for our Hip Fracture ICP Programme.
