Aligned with Healthier SG initiatives, JHF supports community care and outreach programmes and activities that enhance integration of care within the community. 

Anchor and Integrate Care in the Community (AICC)

AICC seeks to reduce acute hospital utilisations and readmissions by providing proactive training and education to nursing home staff, improving their skills, capabilities as well as care transition experience for residents.

Diabetes Reinforcement Empowerment and Motivation Programme (DREAM)

Supporting the ongoing “war” on diabetes, the DREAM Programme adopts a holistic care approach empowering, educating and equipping diabetic patients with self-management knowledge and skills that would improve their health. 

LEARN@ Mobility Park Programme

Learn @ LIFEHub & Mobility Park Programme  supports the members of the public with mobility and safety tips when assisting the elderly and persons with disabilities in Singapore. The programme has since conducted lessons with several Social Service Agencies.

Urinary Catheter Management for Western Region Nursing Homes 

The programme aims to upskill the capabilities of nursing homes in urinary catheter care, reducing incidences of catheter related issues such as catheter-associated UTI (CAUTI) and catheter related trauma; and admissions to the hospital for these issues. 

Spine Care in the Community (SpineCom) Programme

SpineCom aims to right site non-surgical spine patients by co-managing them with NUHS Primary Care Network General Practitioners. Patients receive long-term care and follow-up visits in the community at subsidised rates offered by the Community Health Assist Scheme.
