JurongHealth Campus’ Diabetes ICP Programme aims to empower patients with diabetes with the knowledge to manage their diabetes better. Our multi-disciplinary team – comprising Endocrinologists, diabetes nurse educators , case manager, dietitians, podiatrists, physiotherapists and clinical psychologists – strives to provide patients with the best possible standard of care.
Our programme also aims to build active partnerships with primary healthcare services such as polyclinics and general practitioners in order to provide coordinated and evidence-based diabetic care that is accessible at patients’ convenience.
Only direct referrals from JurongHealth Campus' GP partners will be accepted for our Diabetes ICP Programme.
