Obesity and Metabolic Medicine Service

Overview

Obesity is a chronic, relapsing disease that increases the risk of metabolic and obesity-related health conditions, including type 2 diabetes, cardiovascular disease, sleep apnoea and fatty liver disease.

Achieving and maintaining clinically meaningful weight loss can improve metabolic health, reduce obesity-related complications and enhance overall well-being. Even a modest reduction of 5–10% of initial body weight can lead to meaningful health benefits.

Our Multidisciplinary Approach

The NTFGH Obesity and Metabolic Medicine Service (OMMS) provides comprehensive, evidence-based multidisciplinary care for individuals who are overweight or living with obesity.

The management of obesity is complex and influenced by multiple biological, medical, behavioural, psychological and environmental factors. Our coordinated multidisciplinary team works closely with each individual to develop a personalised care plan that addresses the underlying drivers of obesity while supporting sustainable lifestyle changes and long-term health improvement.

Our multidisciplinary team includes:

  • Doctors experienced in obesity, metabolic health and chronic disease management, who provide comprehensive medical assessment. They evaluate obesity-related health conditions and metabolic risk factors, and discuss evidence-based treatment options, including pharmacotherapy where appropriate. They also provide ongoing medical support to optimise health outcomes.
  • Dietitians who provide personalised, practical nutrition guidance tailored to individual preferences, health conditions and lifestyle. They focus on offering sustainable eating patterns that support long-term obesity management and metabolic health.
  • Physiotherapists / Exercise Specialists who assess physical function, fitness levels and any mobility concerns. They guide safe, achievable physical activity plans that support health, improve strength and endurance, and help patients incorporate movement confidently into their daily lives.
  • Occupational Therapists and other Allied Health Professionals who provide behavioural, lifestyle and psychosocial support. They assist with goal setting, motivation enhancement and overcoming barriers to change. They also help individuals develop sustainable daily routines that support long-term obesity management.

As part of your obesity management journey, some individuals may benefit from additional specialist care based on their clinical needs. This may include referrals to:

  • Gastroenterologists, for the assessment and management of obesity-related gastrointestinal and liver conditions, such as fatty liver disease and gastro-oesophageal reflux disease, as well as the consideration of endoscopic bariatric therapies where appropriate.
  • Bariatric Surgeons, for the assessment of suitability for metabolic and bariatric surgery as part of a comprehensive obesity management pathway.
Your OMMS Journey

Your OMMS journey begins with a comprehensive medical evaluation. Based on your clinical needs, we develop a personalised care plan that may include:

  • Relevant investigations where appropriate
  • Personalised discussion of treatment options
  • Dietetic and nutrition support
  • Physical activity and exercise guidance
  • Behavioural and lifestyle support
  • Ongoing monitoring and review of your progress

Throughout your journey, our multidisciplinary team provides personalised, evidence-based care to support sustainable, long-term obesity management. Follow-up care may include individual consultations, group education sessions and teleconsultations where appropriate, to support ongoing care and long-term success.

As your condition becomes stable, your care may be shared with or transitioned to your primary care doctor for long-term maintenance, with specialist support from OMMS available when clinically required. This shared-care approach supports continuity of care while enabling long-term management closer to home.

    Referral and Eligibility Criteria

    OMMS accepts referrals for adults who meet the following criteria:

    • Aged 18 to 65 years, and
    • BMI ≥ 37.5 kg/m², or
    • BMI ≥ 32.5 kg/m² with one or more obesity-related health conditions, such as:
      • Prediabetes
      • Type 2 diabetes*
      • Hypertension
      • Dyslipidaemia
      • Obstructive sleep apnoea
      • Fatty liver disease
      • Osteoarthritis or other weight-related joint conditions

    *Special Note for adults with Type 2 Diabetes

    OMMS primarily provides obesity management for individuals with:

    • Prediabetes
    • Obesity without diabetes
    • Well-controlled type 2 diabetes who do not require intensive diabetes treatment

    Individuals requiring more intensive diabetes management may be better served in our specialist diabetes clinics, where comprehensive diabetes care can be provided alongside obesity management when appropriate.

    All referrals and appointment requests are carefully reviewed by our clinical team to ensure patients are directed to the most appropriate clinic based on their clinical needs. We aim to provide an update within approximately 2–3 weeks.
