The Renal MDC team comprises a specially trained clinical coordinator, nurse, educator / counsellor, dietitian, medical social worker, pharmacist and specialist clinician. The team will address the specific needs of patients and their caregivers, providing a one-stop and coordinated service for patient's with kidney disease for seamless and houlisitic care.
