Geriatric Medicine provides both inpatient and outpatient specialist care for the elderly aged 75 and above. We receive referrals from intra-hospital departments, polyclinics and general practitioners. Self-referrals may also be made through our outpatient clinics as private referrals.
The elderly often have multiple chronic diseases coupled with functional and psychosocial impairment. Hence, our team will provide a comprehensive geriatric assessment of our patients and adopt a multi-disciplinary approach in the management of their conditions. The care team consists of doctors, nurses and Allied Health Professionals, including physiotherapists, occupational therapists, speech therapists, dietitians, pharmacists, podiatrists and medical social workers where necessary.
We provide inpatient and outpatient palliative care to help improve patients’ quality of life. Our multi-disciplinary team helps patients and their next-of-kin cope with terminal or life-threatening diseases and optimise end-of-life care. We also partner community based services, home hospice services and inpatient hospice units to provide care and bereavement support to patients’ families.
